Mission Statement
Center for Change was incorporated in 2005, with a mission is to provide health education and social services to individuals and families of African American, Hispanic, and Caribbean descent who lack access to economic, social, and health resources.
Description
Center for Change offers a host of health preventive services which includes Chronic Disease Self-management (CDSMP) workshops, Diabetes Self-Management (DSMP) workshops, Health Coaching, and nutrition classes all of which are in English, Creole and Spanish. Our gardening and Medication Therapy Management are English only.
