Description

Elegant Hospice Care provide service to a person and his/her family, wherever they live. This means a patient living in any facility or home can receive specialized visits from physicians, nurses, aides, chaplains, social workers, and volunteers. Elegant Hospice has 24/7 physicians and a dedicated team to discuss the care issues of your loved ones. Elegant Hospice provides services such as non-hospice medical needs at home, telemedicine visits on request, care plan meetings, hospice staff on call 24/7, skilled nursing care, pain and symptom management, disease management, clinical pathway support, assistance with advance directives, caregiver education, emotional and spiritual support, bereavement support and grief counseling, personal care assistance with daily living medical equipment, physical therapy and occupational therapy, medication delivery, and respite care.
