The Case Manager is responsible for providing comprehensive support and guidance to individuals and families enrolled in the Crown Care Essentials program. They work closely with clients to develop and implement individualized service plans. Responsibilities: Assessment: Conduct in-depth assessments to identify clients' strengths, needs, and goals. Service Planning: Collaborate with clients to create individualized service plans that address their unique circumstances. Resource Coordination: Connect clients with essential resources, including food, clothing, hygiene products, and referrals to additional services. Advocacy: Advocate on behalf of clients to access needed services and benefits from other organizations and agencies. Goal Setting: Help clients set and work towards short-term and long-term goals, such as stable housing or employment. Progress Monitoring: Regularly assess and monitor client progress toward achieving their goals. Crisis Intervention: Provide immediate support and intervention in crisis situations, connecting clients with appropriate services. Documentation: Maintain detailed and confidential client records, documenting all interactions and progress. Collaboration: Collaborate with other members of the Crown Care Essentials team, including intake coordinators and community liaisons, to ensure coordinated care.

As you may be aware, mental health issues have become a rising concern among adolescents. The pandemic has made things worse as many young people are struggling with anxiety, depression, suicidal thoughts and other mental health challenges. Unfortunately, many of them do not have access to the professional mental health services they need due to financial constraints, stigma, or lack of awareness. At Crowned for Purpose Organization , we are deeply committed to supporting at-risk youth and promoting mental health awareness in the Metro Atlanta area. We understand that mental health issues can be complex and sensitive, and that each person's experience is unique. We also know that it takes courage to seek help and that no one should have to suffer in silence. That is why we have organized the Walk with Me Sneaker Ball, to raise funds and awareness for adolescent mental health. Our event is not just another charity fundraiser, but a fun and inclusive way to make a real difference in the lives of young people who need our support. We believe that by coming together as a community, we can break down the barriers to mental health care and empower our youth to thrive. Crowned for Purpose Sneakerball 2024 initiative is to raise funds to serve 100 at risk youth by providing mental health services, mentorship, and providing non traditional learning opportunities in the industries of Tech, Trucking, Real Estate and Barber Apprenticeship. Give a man a fish and feed him for a day. Teach a man to fish and you feed him for a lifetime. We are currently seeking volunteers to help with guest registrations, raffles, shoe donation collections, and assistance throughout the event. The event attire is semi formal to formal with sneakers.
