Our mission - why we are here: to improve the health and well-being in partnership with our members, patients, and community. Our vision - where we are headed: health as it could e, affordability as it must be, through relationships built on trust. Our values, what guides our actions are: Excellence, we strive for the best results and always look for ways to improve. Compassion, we care and show empathy and respect for each person. Partnership, we are strongest when we work together and with those we serve. Integrity, we are open and honest and keep our commitments.
We are a hospice program caring for patients at the end of their lives. Our volunteers provide respite for caregivers, companionship to our patients in facilities for socialization and attention, run errands, do light housekeeping, whatever is helpful to enhance the life of a person who is at the end of their lives and support their loved ones. Our volunteers provide much needed respite to caregivers if the patient is in the home, helping them to keep their loved one there and manage the day to day caregiving that can be extremely difficult. A volunteer does NOT provide a medical service, but rather gives some much needed TLC and listening at a very critical time in someone's life. We have been in existence for over 30 years, and the volunteers have always been an essential service that is greatly appreciated and needed by those we serve.
