Care Alliance is a nonprofit, Federally Qualified Health Center that provides primary health care, dental care and supportive services to individuals and families experiencing homelessness, living in public housing, uninsured or underinsured.
Care Alliance provides primary health care and comprehensive oral health services; HIV testing, treatment and counseling; diabetes management; case management; short-term substance abuse and behavioral health counseling; outreach to people living on the street; and women’s health services.
Care Alliance has three primary sites; one strategically located near the two largest homeless shelters in downtown Cleveland, and the other two located within public housing facilities on the near east and west sides of the city. In addition to its three main locations, Care Alliance serves patients at outreach sites which include homeless shelters, drop-in mental health centers and AIDS service organizations (ASO) throughout the City of Cleveland.
In 2009, Care Alliance served 9,000 patients and generated nearly 30,000 encounters. Of the patients served, seventy percent were homeless, ninety-four percent were living below 100% of the federal poverty line and eight-seven percent had no form of health insurance.
