Mission Statement

The My Life, My Story project team believes each veteran has a life story worth telling. Our objective is to give a veteran's medical team the opportunity to get to know patients better and learn what's most important to them in their daily life.

Description

The My Life, My Story project is part of the VA's Diffusion of Excellence initiative and one of only a handful of Gold Status Projects that began at a single VA hospital and later was disseminated to various VA facilities throughout the country.
