Outagamie County Health & Human Services/Volunteers Services coordinates many programs which serve as key support to the work being done by social workers and staff in the department.

When I followed up on why I wasn’t reinstated I learned that the Appleton Social Security office had sent my records to the Wisconsin Disability Insurance Review board in Madison and Outagamie County Health and Human Services a questionnaire that was supposed to be filled out and sent to the Wisconsin disability review board. However, Human Services simply filed the questionnaire in records without filling it out. When I asked a Human Services worker why they didn’t fill out the questionnaire they told me that it was the doctor’s responsibility. However, he doesn’t fill out questionnaires because he doesn’t have the time. When I called the Wisconsin Disability review board in Madison they told me that they can’t make a decision until they get the questionnaire from Outagamie County Health and Human Services
