St. Gabriel’s highly trained and compassionate team members focus not only on the symptom management needs of our patients, but also the spiritual and emotional aspect of end-of-life care. We know that every individual has unique needs and desires, as well as their own story to tell. We are here to listen, to laugh, to love, and to provide care and comfort during this time in our patients’ and families’ lives.
Hospice offers people with a terminal illness an op-portunity for a better quality of life when curative treatment is no longer effective. Hospice treats the whole person, not just the disease; while providing care and support to friends and family as well. The emphasis of care is on helping the patient make the most of every moment by providing comfort and relief from pain and symptoms. Our care team is committed to the ongoing evaluation and implementation of our Physician led Plan of Care. Our focus is on the physical, emotional and spiritual needs of our patients and their loved ones; giving them the care they choose and the compassion they deserve.
Our Physician led Plan of Care is created specifically for each individual patient and managed by our experienced team of professionals, which may include, but is not limited to:
