Our mission is to enable people who are challenged with mental illnesses, intellectual disabilities and/or struggling with addiction to rebuild a meaningful life and thrive in the community.

Description

Continuum of Care, Inc. is a not-for-profit, 501(c)(3) organization that has been providing housing and residential support services to persons with psychiatric and intellectual disabilities since 1966 when it opened as a New Haven Halfway House, one of the first transitional "halfway houses" in the country. Continuum serves approximately 2,000 adults in Connecticut and operates 44 properties.
