Mission Statement
The Health Access and Care Coordination Project is an innovative multi-agency, multi-disciplinary collaborative focused on expanding access and improving coordination across the continuum of care to ensure equitable healthcare for newly eligible Medi-Cal members residing in affordable housing.
Description
The HACC Project was funded by a CenCal Health grant, and includes the Housing Authority of City of Santa Barbara (HACSB), New Beginnings Counseling Center, Santa Barbara Neighborhood Clinics, PathPoint, Doctors Without Walls and DASH. The HACC Project aids to connect HACSB residents with primary care and behavioral health, as well as health and wellness activities. The HACC collaborative provides mobile clinics, clinic hours, home visits with a Nurse Practitioner, health education classes and wellness activities at affordable housing sites throughout the City of Santa Barbara.
