The Healthy-U Disease Self-Management Programs are evidenced-based programs proven to improve quality of life and save money by reducing health care expenses. Adults of any age with a chronic health condition and/or their caregivers meet in small groups once a week for 6 weeks to find ways to solve problems related to common chronic health conditions like diabetes, high blood pressure, heart disease, fibromyalgia, arthritis and others.
Leaders of the program are either adults, preferably with a chronic health condition or professionals such as nurses, care managers, health educators etc.
As a Healthy-U Workshop Leader, you will:
- Receive specialized training and easy-to-use instructional materials.
- Receive free participant workbooks for your classes.
- Experience the satisfaction of changing lives with a proven effective program.
- Facilitate the sessions with a co-leader.
Training classes for Healthy-U Leader Trainees are 4 days. The training is held at COA offices in Tri County but workshop leaders will teach classes in Butler, Clermont, Clinton, Hamilton and Warren Counties.
