Transitions volunteers are trained to provide telephone support, education and community referrals for individuals and families dealing with chronic illness when they are not eligible for the Haven Hospice program. Volunteers wishing to provide direct care services to patients and families must complete a 8 hour volunteer training program, as well as a screening process that includes a reference check, a background check, a TB test, and a personal interview. After training, volunteers are expected to commit to at least 40hours a year. Those who have experienced the death of a loved one must wait at least one year before volunteering with patients and families. Ages 18 and up are welcome.
