Purpose: The overall goal is to equip residents with the knowledge and skills to be able to manage their own lives in a healthy and effective manner once they leave the facility.

The Healthy Living Skills Facilitator needs to have significant experience in leading groups and in working with people who present a variety of physical, mental, and emotional issues.

This position works well with professionals between jobs and professionals who are retired but active.

Mission Statement

The Boulevard: The Road to Health and Home provides a healing environment and resources to help ill and injured homeless adults restore their health, rebuild their lives and regain self sufficiency. The Boulevard is the only agency of its kind in state of Illinois, and one of only a small handful nationwide. Visit our website!


The Boulevard is a medical respite program designed specifically to address the challenges faced by homeless adults discharged from hospitalization with no place to go to recuperate. We aid these individuals in completing their medical recovery or stabilization, begin to address the root causes of their homelessness, and find stable housing. With a capacity of 64 beds (15 women, 49 men), and a usual length of stay of 90 to 120 days, we were able to assist 334 individuals in rebuilding their lives in our last fiscal year.
