St. Joseph Hospice is looking for individuals able to make Quality Assurance/Case Management calls to patients and/or family members on a weekly basis to:
1-See how patients and families are doing
2-Check status of prescription needs and supplies
3-Equipment concerns
4-Follow up with discharged patients to see how they are doing, if they have any concerns or needs.
Calls will be made from our Anaheim office.
