ORGANIZATION: Empath Tidewell Hospice

Transitions is a nonmedical support program for those with approximately a one year life expectancy. Transitions volunteers help clients maintain independence by providing errands and socialization visits, and help caregivers cope with a loved ones illness by providing respite care for them. Regular assignments involve 1-3 hours per week (fewer for social visits, longer for respite visits) and days/times are often flexible.

Founded as Suncoast (1977) and Tidewell (1980) as true pioneers in hospice care, each of our organizations adapted to change and grew to meet the needs of the community, ultimately evolving to become Empath Health and Stratum Health System, respectively.

We came together as Empath Health in March 2021, creating the country’s largest non-profit hospice and home care-based health care organization. Rooted in the remarkable people that give so much of themselves, we look to further our unwavering commitment to uniquely high standards by now growing together.
