Hospice of Cincinnati (HOC) wants to make an intentional effort to understand each patient's story, favorite things and what's most important to them. Why? Because our mission is to create the best possible and most meaningful experience. You can't serve them well, if you don't know them and each patient deserves exceptional care.
To acheive this goal we are starting an initative to create All About Me Boards. We are asking for help to get the base of these boards started so we have them instock ready for HOC Staff and Volunteers can pick up to fill out.
Each board is purposefully designed to look homemade, unique and specially made just for that patient. Pictures can be added to provide recognition and points of conversation. The Doula or HOC Team Member will sit down and complete the board with either the caregiver and/or patient. At a glance, anyone that walks into the room can have access to important information to give that patient a great experience.
Overall, HOC Navigators serve 6,000 patients each year - so we need a lot of help!!!! Our solution is to recruit Community Service Groups to help. Gather your businesses, churches, schools and social groups to do together. Materials can be provided or you can do a collection of paper, stickers and glue to support the effort. HOC will provide a workspace, speaker and facilitator if desired or you can host at your site. If coming onsite each person needs to bring their own pair of scissors, a water bottle, creativity and desire to make a difference! Please contact Lauren_Athey@trihealth.com to inquire and schedule.
