Transition Support Volunteer

ORGANIZATION: Circle of Care, St. Louis

Transition Support Volunteers play a crucial role in assisting seniors relocating from Skilled Nursing Facilities (SNFs) to their new homes. Volunteers will provide emotional support, logistical assistance, and companionship during this significant life change, ensuring a smooth transition for the individuals in our care.**Key Responsibilities:**- **Pre-Move Support:**

  • Engage with residents to understand their needs and concerns regarding the transition process.
  • Help plan and coordinate the moving process, including packing and organizing belongings.-

Day of Transition Assistance:

  • Accompany seniors during the move, providing physical support and guidance.
  • Assist with loading and unloading items, ensuring everything is handled with care.
  • Help set up the new living space to create a welcoming environment.

Emotional Support:

  • Offer companionship and reassurance to ease anxiety and stress during the transition.
  • Create a positive and friendly atmosphere while fostering trust and comfort.-

Follow-Up

  • Check in with seniors post-transition to ensure they have settled in and to provide continued support if necessary.
  • Gather feedback on the transition experience to improve future support efforts.

Qualifications:

  • Compassionate and patient demeanor, especially when working with seniors.
    Strong communication and interpersonal skills.
    Ability to lift and move items safely (physical fitness is a plus).
    Reliable, responsible, and committed to helping others.
    Prior experience with seniors or in caregiving roles is desirable but not required.

Additional Information:

  • Volunteers are typically expected to commit to a few hours during the move day, with optional opportunities for pre-move planning and post-transition follow-ups.
  • Volunteers must pass a background Check and register with MO Family Care Safety Registry Training
  • Volunteer must have a clean driving record
  • Volunteer and orientation will be provided to ensure volunteers are equipped with the neessary skills and understanding to assist effectively.
