Our volunteers are an essential part of the ACG Hospice family, and their support makes it possible for us to provide the meaningful care our patients and their loved ones deserve. There are a variety of ways in which hospice volunteers contribute to our organization with two main type of assistance: direct care, which involves providing companionship with patients in their home or long-term care facility, and indirect care, which does not involve direct patient contact. Volunteers have an immense impact on the lives of our patients and their families, and are truly the "heart" of hospice.

Description
Established in 1997 as "Hospice Care of South Carolina", Agape Care Group (ACG) Hospice's primary vision is to be recognized as a premier provider of care known for our superior patient-family experience, delivering the highest quality care, and building a culture of excellence. Our culture of caring and service reverberates throughout our team, and we are committed to creating an inclusive, respectful environment. In every touch, no matter how big or small, we are dedicated to providing a meaningful patient and family experience.
Our adult hospice care services emphasize the importance of individualized care, ensuring each patient has the right level of support at every stage of their journey. A typical care team for one of our patients could include a registered nurse (RN), certified nursing assistants (CNA), medical social worker, hospice physician, chaplain, grief and loss counselor, and volunteers. Each of these roles are vital in ensuring the best end-of-life care.
ACG is a trusted provider, recognized by multiple 3rd party organizations for our clinical excellence and patient care. We are accredited under both CHAP and ACHC, one of 20 of Medicare-certified hospice providers selected to participate in alpha testing development of the Hospice Outcomes & Patient Evaluation (HOPE) Assessment, and reached our "We Honor Veterans" Partner Level 4 in 2020.
