Mission Statement
Mission: To nurture the healing ministry of the Church, supported by education and research. Fidelity to the Gospel urges us to emphasize human dignity and social justice as we create healthier communities. Core Values: Reverence, Integrity, Compassion, & Excellence Vision: As a ministry of the Catholic Church, we will lead the transformation of healthcare to achieve optimal health and well-being for the individuals and communities we serve, especially those who are poor and vulnerable.
Description
CHI Health at Home is Lincoln’s only 5-star home health agency, offering a holistic hospice service that provides exceptional care to people who are in the last stages of a life-limiting condition so patients and their families can focus on quality of life and what matters most rather than limitations of the disease. The goal of hospice care is to provide the best quality of life possible, through physical, spiritual, and emotional support that respects patients' wishes and ensures peace.
Providing comfort and dignity to patients and their loved ones is what we do. We work with them to create individualized care plans that include specialized services provided by nurses, health aids, chaplains, social workers, bereavement coordinators, and volunteers.
Volunteers are vital to the mission of Hospice and CHI Health at Home, reflecting the compassionate spirit of our ministry by visiting patients and engaging in companionship, crafts & hobbies, memory & keepsake projects, providing emotional and grief support to patients' loved ones, and/or providing office and community support.
