Thanks for your interest in our volunteer program. I will tell you how we work. We visit where our patients live, be it a memory care unit, assisted living facility, personal care home, or their own home. There is travel involved. Once you consent to volunteering and you have done all of your onboarding, you will meet your patient. I will try to match you up with a patient nearest to where you want to visit. The visits we call "friendly visits" are usually one time per week, for one hour or so for you to visit your patient. We do NOT do hands on care with the patient. We can work with your schedule. To be considered as a volunteer you will need to:

1. Fill out the attached application and consent to run a background check - return it to me. Agape Hospice uses Eagle Screen, a third-party background check company. Agape covers the cost. We will email you a form requiring your date of birth and Social Security Number. This is not a credit check and does not have any impact on your credit. The check takes 24-72 hours.
2. Take a TB test. We offer them for free at our Villa Rica or Woodstock office - They can administer TB tests on Monday, Tuesday, Wednesday, and Friday 9-5 you will need to return 2 days later to have the test site read.
3. You will need to take 9 modules online through Relias trainings.
4. You will need to send in a copy of your Driver's license and Auto insurance card.
5. Meet up for an in-person orientation
6. Meet your patient

We also have times where we do community outreach, we go to facilities and call bingo, do crafts, visit veterans. and we always invite all volunteers to help, if you cannot, that's ok, we understand. Our company is unique because we give all volunteers a company email to set up so you can communicate openly with your volunteer coordinator. This ensures that we comply with HIPAA. You will be assigned a company email when you receive your Relias Training information. Agape Hospice Care was voted the best hospice in Atlanta, we are very proud of that. If you want to volunteer with the best group, then Agape is your place. I have a few questions for you to answer:

• What are the days and hours during the weekday that you are available?

• Do you work full time and if so when would you be available?

• Can you commit to seeing your patient through to the end?

• Are you comfortable speaking with people?
• What volunteer opportunities are you interested in?
