Are you a veteran? Would you like to give back to your fellow service members? We are looking for dedicated veteran volunteers to assist with our program and make a meaningful impact in the lives of veterans on hospice care.

As a veteran volunteer, you’ll provide companionship and emotional support to fellow veterans, participate in special pinning ceremonies, and help educate our staff on military culture to ensure the best care and understanding.

Your shared experiences and unique perspective can bring comfort and connection to those who have served.

Mission Statement

The mission of Heartland Hospice is to provide quality hospice care to patients with a limited life expectancy and to support their caregivers. The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of the patients and caregivers. Hospice care is provided by an interdisciplinary team utilizing an approach delivered to patients either in their own home or at an extended care facility. We work cooperatively with health care providers and community resources. Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay. We are affiliated with ProMedica Senior Care.

Description

Heartland Hospice offers support, care, and comfort to the terminally ill and their families. If a individual has been diagnosed with a terminal illness, and can no longer benefit from a curative treatment program (life expectancy of 6 months or less), Heartland Hospice can help. The hospice staff is actively involved in all aspects of the patient's care. This professional staff collaborates with the patient, family, attending physician, and staff (if patient is residing in an extended care facility. The hospice team concentrates on providing comfort, pain and symptom management, spiritual and emotional support. We believe that a good relationship with the patient's health care providers is essential to the emotional and physical well-being of our patients and their families. We develop an individualized care plan which we monitor and update regularly, adjusting it to meet the patient's needs. Both patients and family members are included in this decision-making process. Program features:
24-hour, 7-day-a-week availability and support, individually tailored to meet the unique needs of each patient. Care provided under the direction of experienced licensed nursing staff. A caring and comfortable environment.
A variety of accommodations from which to choose. Pain and symptom management.
Access to spiritual, emotional, and psychosocial support that encourages friends and family to be involved in the program of care.
