The Grandparents Community Network (TGCN) is a nonprofit organization dedicated to supporting grandparents who are primary caregivers to their grandchildren. Through a range of services, including financial assistance, counseling, and resources, we empower grandparents to provide stable, nurturing environments for their families. Our mission is to enhance the lives of grandparent caregivers and strengthen the family unit.

Position Overview:

We are seeking a compassionate and organized Case Manager to join our team. The Case Manager will primarily conduct interviews with individuals and families requesting assistance, assess their needs, and connect them with appropriate services. This role requires excellent interpersonal skills, the ability to work with diverse populations, and a passion for helping families in need.

Key Responsibilities:

  • Conduct Intake Interviews: Meet with clients in person, via phone, or virtually to assess their needs and gather relevant information for assistance requests. Ensure that all intake forms are completed thoroughly and accurately.
  • Assess Client Needs: Evaluate the client's circumstances to determine eligibility for various services, including financial aid, housing assistance, legal support, and caregiving resources.
  • Provide Information and Resources: Offer guidance on available programs and services, including eligibility criteria, application processes, and timelines.
  • Develop Action Plans: Collaborate with clients to develop individualized action plans that outline the steps they need to take to receive support and achieve their goals.
  • Maintained Client Records: Document case details, outcomes of interview intake, and any follow-up actions in the organization’s case management system in a confidential and organized manner.
  • Advocate for Clients: Serve as a liaison between clients and service providers, ensuring clients' needs are addressed and advocating for additional resources when necessary.
  • Follow-Up Support: Monitor client progress, ensure they are receiving the support they need, and follow up regularly to track outcomes and provide additional resources as needed.
  • Collaborate with Other Programs: Work closely with other team members, including social workers, program coordinators, and community partners to ensure clients receive comprehensive support.
  • Participate in Training and Development: Attend regular staff meetings, training, and professional development sessions to stay informed about the best practices and available resources.

Qualifications:

  • Education & Experience:
    • Bachelor’s degree in social work, human services, psychology, or a related field (preferred).
    • At least 1-2 years of experience in case management, social services, or a related field. Experience working with vulnerable populations, particularly seniors or families, is a plus.
  • Skills & Competencies:
    • Strong interviewing and active listening skills.
    • Knowledge of community resources and support programs available to families and seniors.
    • Excellent verbal and written communication skills.
    • Strong organizational skills and attention to detail.
    • Ability to work independently and as part of a team.
    • Empathy, patience, and cultural sensitivity when working with diverse populations.
    • Proficient in Microsoft Office Suite and case management software.

Additional Requirements:

  • Ability to maintain confidentiality and adhere to ethical standards of practice.
  • Flexible work schedule, including occasional evenings or weekends for client meetings.
  • Must pass background checks as required by the organization.

The Grandparents Community Network’s current goal is to provide basic needs, special needs, transitional emergencies and non-traditional circumstances that are not met by community resources or state and federal dollars. We also aspire to provide scholarships for education. We mobilize the collective action of the community sectors to address existing issues, create vital resources, advocate for policies on the local, state and federal level that address the unique needs of grandparents and their families.

Description

We have assisted grandparents in various states with utility assistance, prescriptions, clothing, back to school supplies, car payment assistance and repairs, gasoline, emotional support, crisis intervention and other various nontraditional needs. We currently receive funding from the Founder’s fundraising efforts and personal contributions. Our staff are current volunteers. Our organizational structural consists of 5 board members including the Founder who governs the day-to-day operations and, 1 administrative assistant two volunteer support roles. Our programs consist of mentoring, grandparents' assistance, grief coaching, administrative, radio broadcasting, website design, arts and culture, event planning, workshops and training,
