Holy Cross Home Care and Hospice provides quality care for inidivuals diagnosed with life-limiting illness. Our interdisciplinary team of physicians, nurses, social workers, health aides, chaplains, and trained volunteers work together to provide comfort and care with an emphasis on quality of life. Our volunteers enhance this focus.
A hospice volunteer is someone who provides support and companionship to patients who are in the end-of-life stage. Hospice volunteers work alongside the hospice team to help improve the quality of life for patients by providing emotional support, companionship and assistance with daily tasks such as running errands or light housekeeping. They may also engage in activities such as reading, playing music, or simply listening to the patient's stories and providing a comforting presence. Additionally, hospice volunteers offer respite care for family members and caregivers, allowing them time for self-care or other activities. Hospice volunteers are an integral part of the hospice team, providing compassionate care and support to patients and their loved ones during a difficult time. Description
Founded in 1967, Holy Cross Home Care and Hospice began as a home-based care agency out of the Holy Cross Health system in Silver Spring, Maryland. Our founders saw a need for home-based care within the Silver Spring community and believed our organization could play a significant role in the lives of those who needed our health care services. As one of the early adopters of hospice care in the region, the agency has been at the forefront of home-based services for decades; integrating advanced technology and partnering with colleagues at Holy Cross Health to develop specialized programs.
More recently, in 2024, we expanded our services to include palliative care to help individuals with serious illnesses manage pain and symptoms and improve overall quality of life. As the first Trinity Health at Home agency to move to the Epic electronic health record, Holy Cross Home Care and Hospice empowers patients with efficient access to their health information and other features.
In 2024, we also relaunched Beyond Balance, a fall prevention program for seniors. This comprehensive program includes tailored care plans and patient education, which empowers seniors to thrive at home.
As we move forward, our focus remains to bring compassionate, high-quality care to patients in the comfort of home. In the coming years, we are planning to expand our services and serve more families who need us. Our goal remains the same: to be your trusted home health care provider.CAUSE AREAS
