Sabor y Salud is a new program at Esperanza Health Centers that utilizes food as medicine to address dietary needs that can have an impact on chronic disease. Patients and community members can receive a FREE weekly box of fresh organic fruits and vegetables, this only occurs at this location on Friday Afternoon! Volunteers will stand behind a desk and organize the boxes of produce making sure each box is in good quality, has a recipe paper sheet(provided by us) folded on top and ready to be passed out. One voluteer who has spanish speaking fluency will check in the patient/community member asking for name, phone number, zip code, and the number of individuals living in their hone. The main goal is to provide a welcoming safe space for an individual to gain fresh produce, a place to learn about other resources we offer, and the benefits of including vegetables and fruits into your diet.

Mission Statement

Esperanza Health Centers is dedicated to promoting healthy lifestyles and improving our patients' health through the provision of high quality health care and wellness services in the communities of Chicago's southwest side.

Description

What We Do

At Esperanza, we are dedicated to providing high-quality medical care to all residents of our Southwest Side service area, regardless of their immigration status, race or ethnicity, income level or insurance status. All of our full-time providers and all of our clinical staff are bilingual and bicultural.

We want residents of Little Village (South Lawndale), Pilsen (Lower West Side), Brighton Park, Archer Heights, Gage Park and the suburb of Cicero to come to our bright, sunny offices for all their medical needs.

But we need to do more to have the kind of impact on our community that our residents deserve. That’s why we step outside the clinic, bringing health programs and services to people where they are. All our services and programs are carefully tailored to the needs and culture of the communities we serve.
