We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and support to clients facing various challenges
- Coordinate with healthcare providers, social services, and other agencies to ensure comprehensive care
- Monitor clients' progress and adjust care plans as needed
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date client records
Experience:
- Experience in behavioral health, social work, or related field preferred
- Knowledge of Medicare, Section 8, and childcare services is a plus
- Ability to mentor and supervise individuals effectively
- Proficiency in crisis intervention and motivational interviewing techniques
- Familiarity with addiction counseling principles

Programs / Workshops:
Mentorship - a network of support to help realize their dreams. Through 1:1 career coaching sessions, volunteers help identify their strengths, explore careers, and provide tangible tools to achieve success.
○ Public Speaking Workshop
○ Rich with Attitude GED Program
○ Rich with Attitude Sisterhood Program
○ Mommy and Me Program - sessions, workshops, resources, support groups, teenager support, guidance, how to be a mom, substance use support.
○ Housing Programs - partner with organizations that have housing or case
management and substance use support.
Host events to bring women together. Have guest speakers talk through various trials, tribulations, and their walks through life. Host fundraising events, networking events, etc.
