ABOUT FIVE RIVERS HEALTH CENTERS
The Five Rivers Health Center is an expanding Federal Qualified Health Center located in Dayton and Xenia, Ohio providing a medical home for low-income patients. Our growing organization empowers staff and volunteers to make a difference every day in the lives of others and is committed to providing quality, cost-effective care for our patients and community, with respect and dignity, without regard of ability to pay. Position Overview
The Patient Ambassador will assist Five River’s Health Centers with multiple projects to increase the accessibility and quality of care for all patients. Volunteers will work hand in hand with Patient Access Representatives, Transportation and Facilities team members, and many more.
General Responsibilities
Patient Ambassadors will be responsible for one of following areas:
- Welcome and Registration
- Assist patients with using kiosks.
- Help guide patients to registration desk, other departments, or pharmacy.
- Internal Medicine
- Guide patients to lab, pharmacy, and x-ray from Internal Medicine.
- Assist any lost patients on the second floor.
- Fruit Stocking
- Sort and stock fruit across Edgemont campus building.
- May have occasional other tasks to assist facilities team.
Qualifications
- All volunteers must be 18 years of age or older.

Our Story
Five Rivers Health Centers began on May 23, 2011 as a medical home for the low-income patients of Dayton, Ohio, by providing both primary and specialty care services to patients through the academic rotations for the Wright State University Boonshoft School of Medicine (WSUBSOM) residency and fellowship programs.
Five Rivers Health Centers currently serves more than 27,000 patients with more than 90,000 visits in the Greater Dayton community. We also offer interpreting services to Limited English Proficiency (LEP) patients.
Five Rivers Health Centers consists of three former hospital residency clinic departments of Miami Valley Hospital and Good Samaritan Hospital - Family Health Center, Medical Surgical Health Center, and Center for Women’s Health. Five Rivers Health Centers was approved on April 10, 2012 to be a Federally Qualified Health Center (FQHC) Look-Alike and a FQHC on November 1, 2013. In 2013 we expanded our group even more by adding a Pediatric practice in January, the Five Rivers Dental Center and Pharmacy in September. In August of 2015, FRHC experienced additional expansion by adding the Samaritan Homeless Clinic to our mission. Collectively, these sites provide medical and dental and behavioral health care at eight convenient Dayton locations. In 2017, the Five Rivers Greene County Health Center was added and in 2018, Star Pediatrics was added. Both of these locations are in Xenia, Ohio.
