Description
Thema Health Services uses an interdisciplinary team approach to include the Physician, Skilled Nurse, Social Worker, Home Health Aide, and Chaplain. We strive to maintain patient's wishes concerning their living circumstances. Care is provided in the patients home, caregivers home, group home, assisted living and/or skilled nursing facility.
CHC has 3 major levels of care available to the patient: Routine home care, respite care, and in-patient care as well as providing a coordination of ancillary services and bereavement support.
Hospice affirms life. CHC believes that appropriate individualized care should be provided with a caring attitude. We strive to help prepare the patient, family and caregivers a degree of mental preparedness prior to the patient's death. After the patients death, we follow-up with grieving family members to ensure that all are coping well.
