Considered to be the model for quality, compassionate care for people facing a life-limiting illness or injury, hospice involves a team-oriented approach to skilled care, pain management, and emotional and spiritual support tailored to the patient and family. At the center of hospice is the belief that each of us has the right to die pain-free, with dignity, and on our own terms.

Our Hospice team is dedicated to improving the lives of those with late-stage illness, injury, or disease. We enable patients and families to live life on their terms with more knowledge of what to expect from their disease progression, greater confidence in living life fully in the comfort of their own homes, and the relief that comes from our expertise in pain and symptom management.

About Los Robles Healthcare | Hospice

Location:

509 Marin St., Suite 135, Thousand Oaks, CA 91360, US

Mission Statement

Los Robles Healthcare at Home has been dedicated to providing exceptional healthcare at home since 2000. We believe that everyone is entitled to the highest quality of personalized care and attention - whether you’re recovering from an illness or surgery or looking to live all the days of your life with the support of hospice care. Our belief in total care extends beyond our patients and families to include our healthcare partners, the members of our team, and the community we serve together. That’s why we take a team approach to meeting the physical, spiritual, emotional, and social needs of those under our care, and it’s why we are relentless in the pursuit of our shared vision to set the standard for healthcare in the home.

Description

After all, we understand that no patients are exactly alike - just like no two home health or hospice agencies are exactly the same either. This is at the heart of our commitment to listening to our patients, customizing our care plans to meet their needs, and empowering them to make informed decisions on their own terms ... and in the comfort of wherever they call home.
