The Case Manager is responsible for providing comprehensive support and guidance to individuals and families enrolled in the Crown Care Essentials program. They work closely with clients to develop and implement individualized service plans.

Responsibilities:

Assessment: Conduct in-depth assessments to identify clients' strengths, needs, and goals.

Service Planning: Collaborate with clients to create individualized service plans that address their unique circumstances.

Resource Coordination: Connect clients with essential resources, including food, clothing, hygiene products, and referrals to additional services.

Advocacy: Advocate on behalf of clients to access needed services and benefits from other organizations and agencies.

Goal Setting: Help clients set and work towards short-term and long-term goals, such as stable housing or employment.

Progress Monitoring: Regularly assess and monitor client progress toward achieving their goals.

Crisis Intervention: Provide immediate support and intervention in crisis situations, connecting clients with appropriate services.

Documentation: Maintain detailed and confidential client records, documenting all interactions and progress.

Collaboration: Collaborate with other members of the Crown Care Essentials team, including intake coordinators and community liaisons, to ensure coordinated care.
