The Huron Valley Pace Day in Ypsilanti buses low-income elderly to the Day Center/medical clinic for their all-around well-being. Volunteers socialize with our participants at tables and during craft/game time to increase the value of each individual. Volunteers may also walk participants to the medical clinic or assist with the food pantry. Currently we need volunteers that speak Chinese, Russian, Creole, Spanish and Vietnamese.

PACE is a Program of All-inclusive Care for the Elderly. By providing the services needed we make it possible for low income older adults to live in their own homes and neighborhoods.

Description

Huron Valley PACE provides medical and supportive services in
our Day Center for low income elderly. We also provide transportation
to and from the Day Center. We offer a variety of services that can
be provided in the comfort of your own home. We have relationships
with many specialists and providers in the community who ensure
that participants receive the care they need.

Services provided can include:
* Primary and Specialty Care
* Prevention Services & Education
* Nursing Care
* Social Work & Case Management
* Medications and Medication
Management
* Nutrition Counseling & Meals.
(Breakfast and lunch are offered
at the day center. We also offer
frozen meals to take home)
* Activities, Social Opportunities,
and Outings
* Optometry & Dental Care
* Home Health & Personal Care
* Caregiver Respite
* Hospital & Long-Term Care
