Patient volunteers provide support to our patients and their families in what they would consider their home (eg, residential, nursing home, assisted living facility, etc.)

Patient volunteers provide such support as personal visits, respite time for caregivers, run errands, read to patients, write letters for patients, listening, and caring presence/socialization.

All hospice volunteers receive orientation and training consistent with local, state, and federal regulations.

Mission Statement

Our mission, in honoring life, is to help people of the North Country die in spiritual, mental and physical comfort. We support and encourage people and their families to determine their end-of-life care.

Description

Description

Hospice is about caring for those who are facing the challenges at the end-of-life. Those challenges may be physical, emotional, psychosocial, and spiritual. Hospice cares for the whole person and their loved ones, supporting them with skilled nursing, family services, spiritual support, volunteer services and bereavement and grief support.

Hospice of the North Country offers a wide range of services for patients, their families, and caregivers. We know that each patient’s situation is unique, which is why we offer individualized care for patients with illnesses such as cancer, heart disease, Alzheimer's, or respiratory disease.

Care can be provided at home, or in a nursing home or other facility. Our services are covered by Medicare, Medicaid, Veterans Benefits, and most insurance plans. Our specialized interdisciplinary team consists of nurses, an LPN, social workers, a chaplain, and volunteers-all trained to help you.
