Beth Israel Lahey Health at Home Hospice is looking for volunteers who would like to make check-in calls to home patients and their families before the weekend to ensure they have what they need. It’s an opportunity to connect with patients and families in a different way and offer support. Training is provided.

Description
We offer programs to meet the complex needs of patients and their families coping with advanced illness. These programs include palliative care, bridge-to-hospice, hospice at home and residential hospice. The unique aspect of our care is that the bridge and hospice programs are coordinated by the hospice team, which assures that patients receive care from a care team with extensive experience and training in managing symptoms. This care team remains the same as patients transition to different levels of care.
