PACE stands for "Program of All-Inclusive Care of the Elderly". The Bluegrass PACE Care Program helps people ages 55 and up meet their health care needs and avoid going into a nursing home. To learn more about PACE click here: https://www.youtube.com/watch?v=w4rUG0H1DXs
How can you make a difference at the PACE center?
- Socialize with participants.
- Help with crafts.
- Assist with recreational activities like calling Bingo.
Volunteers are needed at 1 pm on Wednesdays to help with recreational activities.

Bluegrass Care Navigators is innovative in expanding or evolving services to meet the care needs of its communities.
Our current services include:
Extra Care - personalized home care and private duty
Home Primary Care - primary care for "home limited" patients
Adult Day Health Care - day program for adults with physical or mental conditions
Transitional Care - help with hospital to home and reduction in readmission
Palliative Care - physician consult service for pain and symptom relief
Hospice Care - expert interdisciplinary care for life’s final months
Grief Care - counseling service for anticipatory grief and bereavement
