Volunteers are residents living in the areas where we serve. They include people who are retired and those currently working. They range in age from teens to senior citizens. People who want to volunteer for Hospice do not need special talents; they simply need to have the desire to "make a difference in people’s lives".

Making a Difference Hospice care is not about giving up ... it is about hope for people facing a life-limiting illness - hope of living their lives free from pain, without fear and in the company of family and friends. The emotional and spiritual support that Trinity Hospice provides to patients and their families is greatly enhanced by the help of our volunteers.

Volunteers are an invaluable part of our interdisciplinary team. They demonstrate a deep commitment to the Hospice philosophy and to their desire to help others.

What do Hospice volunteers do? Our volunteers assist patients and their families in a variety of ways, including:

- Reading
- Sitting quietly with a patient
- Playing music for patients
- Administrative work in the Hospice office
- Assistance with errands, transportation and household chores

The most valuable skill a volunteer has is the ability to be a compassionate presence. Volunteers increase the quality of life by offering emotional support and companionship to patients and their families.

Mission Statement

Autumn Hospice's mission is to provide dignity and quality of life through death and beyond with compassion, passion and commitment. Focus of care is directed toward patient goals for the remainder of life and is adjusted and refocused as the condition of the patient changes. Nurses, social workers, home health aides, volunteers, bereavement counselors, chaplains, hospice physicians and management of Autumn Hospice provide a dynamic, holistic interdisciplinary team to serve the patient, family, caregiver and significant other which is recognized as the unit of care." The Autumn Hospice team will continuously strive to meet and exceed the unique and very individualized spiritual, emotional, and physical needs of the "patient/unit of care" during the dying process, continuing through the bereavement (thirteen months) period that follows the death of the patient.

Description

Hospice care is patient centered and patient directed. It is one of the best examples in health care of collaboration by an interdisciplinary team dedicated to meeting the needs of the patient and family. In consultation with the patient, his or her loved ones, primary caregiver, and the patient’s own physician, the hospice team prepares an individual plan of care based on patient and family needs and goals. Direct care is provided by the primary caregiver, if available, and augmented by the hospice team. Care is provided wherever the patient calls home.

Hospice also provides medications related to comfort and the terminal diagnosis, some supplies, and durable medical equipment needed. Hospice is designed to support the care that the patient’s primary caregiver provides.

The Autumn Hospice team works together with you to develop a plan of care that meets your needs and provides you and your caregivers with care and support. The following are the services offered by each of the team members:

  • Any time you can contribute! Whether it be an hour a month or a few hours every week any time used to provide volunteer services for our patients is valuable. Let us know what your schedule is and your interests are and we will always work with you!
