Community Wellness Program Leaders are needed to facilitate the following evidenced based programs in their communities:
- Diabetes Prevention Program
- Living Well with Hypertension
- Walk With Ease
- With Every Heartbeat is Life
Volunteer Requirements
Comfortable presenting for, and encouraging discussion in groups
Access to reliable transportation
Availability between 9 am -5 pm weekdays
Commitment to leading at least three workshops per year and attend training
Individuals with chronic illness are especially encouraged to apply!
