Circle of Care Volunteers call home care patients or their caregivers each week to check on their
needs prior to the weekend. Volunteers follow a phone script, assisting them to identify patient needs such as medication refills, supplies and/ or equipment that need to be addressed prior to the weekend. Circle of Care Volunteers contribute to a proactive approach in caring for terminally ill patients and their families. After training and education, the calls may be done from home or from the AccentCare office.

Description
Our Hospice services include pain and symptom management, access to nursing and team support 24/7, medications, supplies and equipment related to patient symptoms, bereavement counseling, patient and family education, end of life planning and emotional and spiritual support. Our specially trained hospice team includes an Attending Physician, Nurse Case Manager, Medical Director, Hospice Aide, Bereavement Coordinator, Spiritual Care Coordinator, Social Workers and Volunteers.
