About Trinity Hospice

545 SW Cutoff, Suite 9, Worcester, MA 01607, US

Mission Statement

Trinity Hospice's mission is to provide/improve the dignity & quality of life through death and beyond with compassion, passion & commitment. Focus of care is directed toward patient goals for the remainder of life & is adjusted & refocused as the condition of the patient changes. Nurses, social workers, home health aids, volunteers, bereavement counselors, chaplains, hospice physicians & management of Trinity Hospice provide a dynamic, holistic interdisciplinary team to serve the patient, family care giver and significant other which is recognized as the "unit of care." The Trinity Hospice team will continuously strive to meet & exceed the unique and very individualised spiritual, emotional and physical needs of the "patient/unit of care" during the dying process, continuing through the bereavement (thirteen month) period that follows the death of the patient.

Trinity Hospice is dedicated to improving the quality of life through compassionate care for individuals and families facing an end-of-life illness. Our staff of highly qualified physicians, nurses, certified home health aides, social workers, clergy, and volunteers work in partnership with the patient, family, and their personal physician to make the final period of life comfortable and peaceful.
