Volunteers at The Care Team Hospice often spend their time sharing stories with the people we care for while keeping them company, wherever they live. We welcome all. If you play an instrument, like to read stories, do crafts, and have a big heart, this is the opportunity for you!

We are a collective team of healthcare professionals and volunteers that desire to provide premier end of life care to those who have been diagnosed with a life limiting illness. We provide a wholistic approach in collaboration with the patient’s circle of support.

Description

Hospice - or "comfort" - care is used to describe an approach to healthcare that is specifically designed to support the physical, emotional and spiritual needs of those who have life-limiting illnesses or conditions.

Hospice can be offered in a patient’s home or a care facility. It is designed to provide emotional support to patients and families, and to educate caregivers so that they are able to provide physical care to their loved one. Hospice care provides compassionate care and support for those in the last stage of life so that they may live as fully and comfortably as possible.

Hospice is a physician-directed, nurse-coordinated, interdisciplinary approach to patient care. Hospice team members can include a registered nurse, a medical director, a home health aide, a social worker, a spiritual care coordinator, a volunteer and your attending physician, if he or she remains involved in your car
