ProMedica Hospice (formerly known as Heartland Hospice) is a nonprofit organization which provides quality care to patients with a limited life expectancy, as well as support to their families and caregivers. We enhance the quality of life of those we serve through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of our patients and caregivers. Hospice care is provided by our caring and knowledgeable interdisciplinary team -- either in the patient's home or extended care facility --while working cooperatively with other health care providers and community resources. ProMedica Hospice strives to provide care to all who are eligible and need hospice care, irrespective of ability to pay.

ProMedica Hospice serves Clinton, Eaton, Hillsdale, Jackson, and Ingham counties in Michigan. ProMedica is a nonprofit organization.
Hospice provides specialized care, support, and comfort for patients in their final stage of life - when they can no longer benefit from a curative treatment program. Our dedicated team is actively involved in all aspects of our patients' care, collaborating with the patient and their family, attending physician, and additional caregivers to provide pain and symptom management, as well as spiritual and emotional support.
Our team at ProMedica Hospice is comprised of interdisciplinary professionals including our Medical Director/Physician, Nurse Practitioner, RN and LPNs, nurse aides, and ancillary support services such as Bereavement, Spiritual Support, Music Therapy, Massage Therapy, and our administrative office staff.
ProMedica Hospice utilizes volunteers to provide further support to patients and families in the form of friendly visits, pet or respite visits, or by volunteering in our office to help with administrative tasks such as letter writing or filing documents.
