Evergreen is a comprehensive health care delivery system that provides medical, supportive and behavioral health services to individuals and families in Western NY- especially those who are living with a chronic illness or who are under served by the health care system. The EOC member would be working alongside the Medicaid Redesign Team (MRT) Housing Team. The MRT Housing Team works with people who are living with chronic health conditions and who are experiencing homelessness. The team works towards assisting underserved individuals with finding and maintaining safe & affordable housing, while ensuring their mental and physical health needs are also being addressed. Due to the high-need patients served in our program, we often come across many who have no or very limited income while trying to navigate the housing market. The EOC member would specifically be trained in housing navigation services and SSI/SSDI Outreach, Access, and Recovery (SOAR) programming.
