Beth Israel Lahey Health is here to drive the change Massachusetts wants and needs. We’re doing things differently, but it’s more than that. We’re here to help our patients, their families, our communities and health care as a whole, be healthier.
We offer programs to meet the complex needs of patients and their families coping with advanced illness. These programs include palliative care, bridge-to-hospice, hospice at home and residential hospice. The unique aspect of our care is that the bridge and hospice programs are coordinated by the hospice team, which assures that patients receive care from a care team with extensive experience and training in managing symptoms. This care team remains the same as patients transition to different levels of care.
