PIH Health considers patients and families as partners and active participants in advanced care planning. The purpose of this volunteer role allows the volunteer to facilitate this process by providing Advanced Care Planning education with the goal of the patient completing an Advanced Directive or Physician Orders for Life-Sustaining Treatment (POLST). The volunteer will have a conversation with the patient about their hopes, goals and wishes for the future and will identify issues and opportunities which have the potential to improve care and services. The volunteer will also connect the patient with the appropriate resources as needed, such as Chaplain, Nursing, or Social Services. The volunteer may answer questions about Advanced Directives/POLST and review the form, but will not complete the Advance Directive form for the patient.

Description
Volunteer Services is currently seeking adult and student volunteers. We currently have 3 categories of volunteers: High School, at least 16 years old and a Sophomore, College and Adults, adults, primarily not in school. When applying, please indicate which category you are.
