CommunityHealth is a volunteer-driven free health clinic serving low-income, uninsured patients in Chicagoland.
At CommunityHealth, we rely on generous volunteer medical providers to ensure access to high quality care at no cost to our patients. We are seeking primary care providers (MD/DO/PA/NP) to staff our new locations in Little Village and Belmont Cragin, serving either onsite or from home via telehealth.
As a primary care provider at CommunityHealth, you have the opportunity to care for patients in a supportive and collaborative setting. CommunityHealth offers comprehensive services, allowing you to refer patients to over 20 in-house specialties (including dental), order labs and medications, and engage with health education and behavioral health departments.
CommunityHealth's new locations are situated within local social service agencies, allowing us to bring care closer to home and collaborate more closely with organizations addressing social determinants of health.
Volunteer Spanish and Polish interpreters are available for all appointments, and of course Spanish and Polish-speaking providers are very welcome!
