We are looking for healthcare providers that are interested in working with the Farm-Based Wellness Program (FBWP) Participants this summer as session leaders/co leaders during the nutrition discussion of the program. The FBWP runs from 5:30 pm - 7:30 pm on Tuesday (Spanish) & Thursday (English) evenings from 5/17/22 up to 9/01/22. The nutrition discussion starts after the meditation session and runs for one hour, from 6:30 pm - 7:30 pm. The program uses the National Diabetes Prevention Program (NDPP) 2012 as the foundation for its curriculum. The FBWP includes modifications that help promote whole-food plant based nutrition to its participants.

On Tuesdays the healthcare provider should be comfortable to instruct a predominantly Spanish group, however a translator will be on site each day. An additional program day is offered to participants to engage in group physical activity and to volunteer on the Ferry st. Farm. Healthcare providers and other stakeholders are invited to join us during the Saturday sessions if they would like to engage in group physical activities as well - no sign-up necessary. The program will take place at the following locations within the city of New Haven: Tuesday: 15 James st., Thursday: 170 Ward st. & Saturday: 613 Ferry st.

The goal of the FBWP is to provide participants that have been referred to the program by their healthcare provider with an experience that empowers them to manage their food-related illnesses more effectively. The FBWP aims to work towards creating an alternative model of healthcare that promotes healthy lifestyle changes and functions as a form of preventative care to improve the health outcomes of participants while reducing the burden of preventable disease on healthcare facilities.

This position requires a one time commitment of 1 hour (6:30 pm - 7:30 pm) on the day of the session.

Supervisors: This position will be supervised by the program manager, any questions or concerns regarding program implementation will be directed toward the program manager, Peter Zapata. This is a volunteer position sponsored by Gather New Haven. Any questions or concerns regarding financial reimbursement will be directed toward the executive director, Brent Peterkin.

Dates & times: Healthcare providers can sign up for specific session dates according to availability and interest. Leader/Co-leader are for weekday programming. Healthcare providers are welcome to join us for Saturday programming but it will be focused on physical activity and farm volunteering. FBWP participants will have their orientation on the 10th and 12th of May for Spanish (Tuesday) and English (Thursday), respectively. Providers and other stakeholders are welcome to join us! The first week of programming (session 1) is the following week, the 17th & 19th of May during the weekdays; the 21st will be the first Saturday program.

Farm-Based Wellness Program: The Farm-Based Wellness Program (FBWP) is an intensive lifestyle intervention program that primarily receives patient referrals from local clinics in the New Haven area with diet-related illnesses to participate in a 16 week summer program where they are engaged in gardening activities, mindfulness, nutrition education, and a whole food plant-based nutrition recipe demonstration. The program continues throughout the remainder of the year on a monthly basis for participants to continue to receive support and motivation to reach their self-contrived nutrition and physical activity related goals. After the year is over, participants are encouraged to stay engaged through social media and/or community events. They are also encouraged to cultivate food at one of their local community gardens.
