Only 33% of Veterans access any benefits. Veteran benefits include healthcare, burial, survivor, aide and attendant, memorials, honors, tax deductions, college savings, etc. Our Veteran Resource Volunteers will be assigned to patients who are military Veterans and help ensure the patients are informed and educated about all entitled benefits. Additionally, these volunteers may be asked to assist with the process of replacing lost military medals and requesting honors. I encourage anyone who is interested in learning more to reach out. Whether or not you decide to proceed with this role, the education is valuable for anyone who would like to know more about supporting your loved ones who are Veterans.

Mission Statement

The mission of Heartland Hospice is to provide quality hospice care to patients with a limited life expectancy, as well as, support to their caregivers. The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of the patients and caregivers.

Description

Hospice care is provided by an interdisciplinary team consisting of the Medical Director, Primary Physician, Nurses, Social Workers, Chaplains, as well as Bereavement Coordinators and Volunteers. Care is delivered to patients either in their own home or extended care facility. Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay.
