At Affinity Hospice, we believe in providing specialized care to the patient and support for the family unit as a whole by meeting their physical, emotional and spiritual needs in the case of an advanced illness. Our goal is to enable patients to live alert and pain - free as possible while encouraging personal choices so days can be filled with dignity, quality, and shared with friends and loved ones.
Description
Affinity Hospice provides care wherever the patient may live across the 12 Counties that we cover. Whether it is in a home setting, an assisted or independent living community, or a skilled nursing facility. We believe in the quality of life and are committed to our patients to live life as alert and pain free as possible with love and compassion.
At Affinity, our Hospice becomes a multifaceted support system to patient and family, arranging every aspect of care in regards to the illness---physical, emotional and spiritual needs. Our care includes comforting care by nurses, physicians, social workers, certified nursing assistants, clergy, and volunteers. Our physician at Affinity is available 24/7, which greatly reduces or eliminates the need for emergency room visits or hospitalizations.
Our volunteer program stresses human values that go beyond the physical needs and is just as important as the rest of their care. Our volunteers play a vital roll by offering non-medical support to the patient and their family members through assistance with tasks, companionship to so much more so our patients can live their final days with dignity, comfort and extra love they need.
