Description
Heritage OnCare provides Hospice care wherever the patient calls home - private residence, nursing home or assisted living community. In collaboration with a patient's physician & Hospice Medical Director, we focus on quality of life using Expert medical care, pain & symptoms management, and emotional & spiritual support.
To care for those who once cared for us is one of life's highest honors. While you are serving others in your role, we want you to know that YOU are the most important asset we have. We could build the most beautiful communities in the world but without our associates and volunteers everything would fall short. Our team members have the distinct privilege of building relationships with those we serve.
Heritage Communities was founded by Farhan Khan, CEO & Nate Underwood President, with the first community, Shalimar Gardens opening in Fremont Nebraska in 2000. Over the past 20+ years we have grown to more than a dozen communities in four different states with over 1,200 associates and volunteers all working toward the same mission of Living Better and upholding the same core values of trust, respect, compassion &fun.
Heritage OnCare is a partner of We Honor Veterans, a program of the National Hospice & Palliative Care Organization. As a partner, we are privileged to care for and honor those who served our country.
