Become an i mportant Care Team Member: Make Calls to support patients and families in Hospice Care
Responsibility includes: Making weekly calls to Hospice home patients to make sure they are comfortable and have everything they need. Your compassionate call will reduce patient/family anxiety about running out of much needed medications and supplies. The goal is to improve overall patient care, caregiver confidence, increased support to Bluegrass Hospice Care team members and to increase family satisfaction.
Commitment: Approximately, 2 hours every Thursday morning from 9am - 11am (timing flexible). Calls can be made from the Hospice office or volunteer's home. Thorough training and staff support is provided.
Requirements: Must have some knowledge of phone operation, good communication skills to compassionately communicate with patients and families and dedication to the ongoing weekly calls.

Transitional Care - help with hospital to home and reduction in readmission
Palliative Care - physician consult service for pain and symptom relief
Hospice Care - expert interdisciplinary care for life’s final months
Grief Care - counseling service for anticipatory grief and bereavement
