ORGANIZATION: UHPhealth

UHPhealth is looking for a Registered Dietitians who will serve as an expert in the use of nutrition to promote a healthy lifestyle. They will provide appropriate patient nutritional care, assess the nutritional status of patients and provide a wellness plan that’s customized to individual's need, and goal appropriate for the age of the patient. Provide nutrition education to patients/families.

Duties and Responsibilities:

· Screening, assessment, reassessment and medical nutrition therapy

· Meet with each patient on a weekly basis to implement and manage individual dietary meal plans including conducting weekly calorie counts.

· Facilitate Nutrition Group that are appropriate to the age-appropriate patient.

· Complete documentation on each client's progress throughout the treatment experience, making sure that daily progress note reflects the goals within the treatment plan.

· Implement one dietary treatment goal for each client within a week of admission.

· Communicate with treatment team throughout patient's treatment episode, Facilitate family sessions and education; including virtual therapeutic meal; as applicable.

· Weekly communication (phone, email, or in person) with parents of adolescent. Provide age-appropriate education group to support the nutritional plan of care.

· Assess educational needs and readiness to learn for all patients and families.

· Collaborate with another professional team to integrate and communicate nutritional principle that will enhance the patient outcome.

· Complete activity and other report on timely basis.

· Identifies and documents evaluation of outcomes.

· Performs other duties as assigned.

UHPhealth is a member of the Texas Association of Charitable Clinics, and delivers affordable, accessible, community-based health services and education to underserved and uninsured communities. We strive to support the well being of our clients and have been providing care for less privileged and underserved communities since 2015.

At UHPhealth, we believe living involves all aspects of a person’s life. Our programs are aimed to serve families and individuals and improve social development and their overall well-being.

Being a Charitable Clinic means we operate with little to No state or federal funding. We are able to deliver quality care to the community with the help of staff members, including licensed physicians and nurses as well as clinical and non-clinical team members who volunteer their time.
