ORGANIZATION: FOSTERING EMPOWERMENT THROUGH CANINE HELP

These are essentially the final items before we can search for the 1st recipient of one of our psychiatric service dogs so the sooner we can get these items handled the better. Here are the items that we are looking for legal review assistance on. Note that ALL these items have already been created - we are just looking at assistance on review and feedback to protect ourselves, our clients, our trainers, the service dogs, and our organization:

  • Brief Application Process - we have a very rudimentary explanation of what we were thinking the application process might look like. Wanted to have someone review what we plan to have this process look on a high level in case this volunteer with a legal background sees something wouldn't be correct or considered "legal" for us to do.
  • Application - we have a full application we created in Google Forms that we plan to post on our website for women who would like to be considered for the funds for a psychiatric service dog from our organization
  • Consent/Acknowledgment - this would be signed off on (either via Google Forms if we can figure that piece out OR in a separate DocuSign style link/document) at the time the application is completed
  • Psychiatric Physician Questionnaire - once we receive the application and consent and review with our trainers to see if we can meet the needs or if they would make a good applicant, we would send this on to the Psychiatric Physician that the candidate provided us contact information for in their application

Mission Statement

OUR MISSION: To provide a highly trained psychiatric service dog to women who have endured physical or mental abuse so that they are no longer merely surviving but thriving, regaining or building an independent and fulfilling life. Our fundraising efforts will be used to provide and train these service dogs through our partnership with licensed service dog trainers as well as provide post-placement tools to facilitate a successful transition for the women we serve as they transition to life with a service dog.

Description

Provide Financial Assistance for women to obtain psychiatric service dogs from our dog trainer partners who meet the following criteria:

  • Have been physically and/or mentally abused, resulting in life with debilitating psychiatric conditions
  • Have been seen by a Psychiatric Physician who determined they are eligible and would benefit from a psychiatric service dog due to their condition
  • Have a financial need for assistance to obtain a service dog
  • Are located in the Dallas/Ft. Worth area
